Nurse Practitioner

We are seeking a compassionate and skilled Nurse Practitioner to join our team in providing high-quality care to patients with serious illnesses. As a Hospice and Palliative Care Nurse Practitioner, you will work closely with a multidisciplinary team to deliver comprehensive care that addresses the physical, emotional, and spiritual needs of our patients and their families.
Responsibilities- Provide primary and specialty care to patients with serious illnesses, including those with cancer, heart disease, and other life-limiting conditions.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Conduct thorough assessments, diagnose conditions, and develop treatment plans that prioritize patient comfort and quality of life.
- Prescribe medications and therapies as needed to manage symptoms and alleviate suffering.
- Provide emotional support and counseling to patients and their families, addressing their concerns and fears in a compassionate and empathetic manner.
- Stay up-to-date with the latest research, guidelines, and best practices in hospice and palliative care, applying this knowledge to improve patient outcomes and quality of care.
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
- At least 2 years of experience within the last 5 years in chronic disease management/long-term care, with a preference for hospice background.
- Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 25 lbs.
