Compensation PackageWe offer a highly competitive compensation package, including a base salary of $60,000 - 80,000 per year, depending on experience. In addition to the base salary, you will receive a comprehensive benefits package, which includes:

This role is a dynamic opportunity for a skilled Registered Nurse (RN) to join our team as a Home Health and Hospice Caregiver. As a key member of our interdisciplinary team, you will be responsible for coordinating and facilitating patient care plans, assessing patient needs, and collaborating with physicians and other healthcare professionals to ensure seamless care delivery.
Responsibilities and RequirementsThe successful candidate will possess excellent communication skills, a strong commitment to patient-centered care, and the ability to work effectively in a fast-paced environment. Key responsibilities include:
- Assessing Patient Needs and Coordinating Care Plans
- Providing Skilled Nursing Care and Education
- Collaborating with Physicians and Other Healthcare Professionals
- Documenting Visit Notes, Treatments, and Care
- Maintaining Accurate Records and Adhering to Regulatory Requirements
To succeed in this role, you must possess a current Registered Professional Nurse (RN) licensure in the state where you will be working. Additionally, you should have basic life support training and a valid driver's license. Work Schedule and LocationThis full-time position offers a 32-hour weekly schedule, with flexibility to accommodate changing patient needs. You will be required to travel to patient homes, community-based facilities, and other locations to provide care and education.
