Job DescriptionWe are seeking a highly skilled Contract Therapy Coordinator to join our team at One Home Health LLC. The successful candidate will be responsible for managing, supervising, coordinating, evaluating, and developing client care teams to ensure quality care delivery and appropriate case management by all third-party therapy providers the Agency has contracted within scope of services and policies; state, federal, and local laws.Key Responsibilities:
  • Manage and direct contract therapy staff of contracts that the Agency utilizes to care for patients.
  • Monitor all patients referred to contract companies, approve care plans, monitor utilization and any changes to frequency and/or orders, and coordinate appropriate discharge dates.
  • Maintain communication with directors/managers and field staff of contract companies.
  • Review and approve contract therapy invoices for payment to ensure all visits were performed by Agency's standards.
  • Provide support and direction to contract staff.
Requirements:
  • Current license to practice as a Physical Therapist or Physical Therapist Assistant.
  • Minimum of one year of home health experience.
  • Complies with accepted professional standards and practice.
  • Excellent verbal and written communication skills and strong interpersonal skills.
  • Knowledge of home care federal and state regulations.
  • Knowledge of reimbursement sources and documentation requirements within home health care.
  • Possess basic computer skills.
