Job Summary

We are seeking a skilled LPN Home and Community Care Specialist to join our team at PeaceHealth. As a key member of our home health team, you will provide high-quality patient care in a home setting, working closely with our patients, families, and healthcare providers to deliver exceptional outcomes.

Key Responsibilities
  • Implement and evaluate patient care plans under the supervision of a registered nurse.
  • Provide direct patient care in patient homes, including administering medications and treatments as prescribed.
  • Conduct focused systems and wound assessments, and report acute medical problems and condition changes to the case manager and/or attending physician, as directed.
  • Communicate effectively with individuals diagnosed with mental and behavioral disorders, and operate efficiently in an independent home setting.
  • Document patient care in health records according to department standards.
Requirements
  • Graduate of an accredited State Board of Nursing LPN Program.
  • Registered as a LPN with the state of Oregon.
  • Current Basic Life Support (BLS) certification issued by the American Heart Association.
  • Valid Drivers License and proof of Insurance.
