PeaceHealth is seeking a Licensed Practical Nurse (LPN) for Home and Community Health Services. This is a Part-Time role, requiring a commitment of 0.50 FTE during Day shifts. The salary range for this position is approximately $41.21 per hour.
The actual compensation will be determined based on various factors, including job responsibilities, location, collective bargaining agreements, education, training, work experience, seniority, and performance.
Hiring bonuses and relocation assistance may be available.
Key Responsibilities:
Under the guidance of a Registered Nurse (RN), the LPN delivers direct nursing care within the scope of their licensure, following directives from physicians and other healthcare professionals. Collaborative decision-making is essential in fulfilling job duties.
- Implements and assesses patient care plans under RN supervision.
- Engages in the creation of individualized care plans for patients.
- Provides direct care in patients' homes, including administering prescribed medications and treatments.
- Demonstrates knowledge in managing edema/lymphedema, performing venipuncture, and handling PICC lines, implanted ports, post-surgical drains, and both pulmonary and abdominal drains.
- Conducts focused assessments of systems and wounds, reporting any acute medical issues or changes in condition to the case manager or attending physician as necessary.
Qualifications:
- Graduate of an accredited LPN program recognized by the State Board of Nursing.
- Licensed as an LPN in the state of Oregon.
- Current Basic Life Support (BLS) certification from the American Heart Association.
- Valid Driver's License and proof of insurance.
