About the Opportunity:

Children’s Healthcare of Atlanta invites applications for the position of Behavioral Health Patient Navigator. This crucial role requires a highly motivated and compassionate individual to lead the care coordination process for our Behavioral Mental Health population.

About the Job:

You will work closely with our multidisciplinary teams to develop and implement personalized transition care plans, ensuring seamless transitions for our patients and their families.

Key Responsibilities:

  • Conducts comprehensive assessments of patients upon admission to identify needs related to care coordination and/or discharge planning.
  • Collaborates with clinical teams to develop and implement robust transition care plans tailored to each patient’s needs.
  • Maintains open lines of communication with patients, families, and healthcare professionals to address any emerging concerns or barriers.
  • Coordinates with home health agencies, third-party payors, and community resources to ensure efficient discharge processes.
  • Acts as a liaison between hospital departments and external organizations to promote collaborative care planning.
  • Continuously evaluates and refines care coordination strategies to improve patient outcomes.

About the Salary:

The estimated salary for this position is $63,500 - $83,000 per year, based on experience and qualifications.
