We are seeking a highly skilled Nurse Navigator to join our team at Insight Global.
About the Role:This is a unique opportunity to work in a dynamic, fast-paced environment as part of our Medical Command Center (MCC) team. Our MCC utilizes a national network of care coordinators and communication centers to provide world-class care coordination and patient navigation services. As a Nurse Navigator, you will play a critical role in connecting patients with the right resources and support they need.
About the Job Description:In this role, you will be responsible for triaging patients over the phone according to established guidelines, navigating care to appropriate facilities across the country, and providing remote telemonitoring for patients nationwide. You will also participate in Quality Improvement activities and work closely with our team to ensure seamless patient care.
Requirements:To succeed in this role, you must have an active LVN or RN license without restriction, graduation from an accredited nursing school, and experience with telephonic assessing and remote patient monitoring. You should also be comfortable handling up to 35 calls per shift, including charting, and possess excellent multitasking and adaptability skills. Additionally, you should have knowledge of clinical practices, procedures, and protocols, as well as experience working with all patient ages, from pediatric to geriatric.
Estimated Salary:$60,000 - $80,000 per year, depending on location and experience.
