Hospice Care Nurse

**Overview of the Role**
At Fortis Health, we are seeking a skilled Hospice Registered Nurse to join our team. This is a full-time position that offers a competitive salary and benefits package.
The successful candidate will be responsible for planning, coordinating, and providing care for patients in their homes. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
**Estimated Salary:** $85,000 per year
**Responsibilities:
- Conduct thorough assessments of patients' needs and develop individualized care plans
- Collaborate with other healthcare professionals to ensure seamless care delivery
- Provide education and support to patients and their families on pain management and symptom control
- Maintain accurate records and documentation of patient care
- Participate in regular case conferences to discuss patient progress and plan for continued care
**Requirements:**
* Graduate of an accredited school of nursing
* Current state licensure as a Registered Nurse
* One year of acute care experience within the last two years, or one year of home healthcare experience within the last two years, or credentials in specialty area (e.g., phlebotomy, IV, ET)
* Current CPR card and valid state driver's license
* Proof of current automobile insurance
