Job Responsibilities:

  • Conduct thorough assessments of patients' physical, emotional, and spiritual needs
  • Develop and implement individualized care plans tailored to each patient's unique needs and goals
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and achieve optimal patient outcomes
  • Provide education and support to patients, families, and caregivers on disease management, medication adherence, and end-of-life care
  • Maintain accurate and timely documentation of patient progress, including vital signs, lab results, and treatment plans
  • Participate in regular case conferences to review patient progress, address concerns, and adjust care plans as needed

Requirements:

  • Degree in Nursing (RN) from an accredited institution
  • Current state licensure as a Registered Nurse
  • Minimum 1 year of experience in hospice or palliative care, home health, or acute care settings
  • Excellent communication, interpersonal, and problem-solving skills
  • Ability to work independently and as part of a multidisciplinary team
