Job Summary:
Healthcare Support is seeking a skilled RN Hospice Case Manager to join our team. As a RN Hospice Case Manager, you will be responsible for providing high-quality care to patients in their homes. This is a 13-week contract position, working 40 hours per week, with a shift of 8 hours.
Responsibilities:
- Provide comprehensive care to patients in their homes, including assessing their needs and developing individualized care plans.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Conduct regular home visits to monitor patient progress and adjust care plans as needed.
- Document patient information and care plans accurately and thoroughly.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements:
- At least 12 months of hospice experience in home settings within the past 3 years.
- Experience working in Home and Community-Based (HCHB) settings.
