About the Role:As a Registered Nurse Case Manager, you will be responsible for assessing patient needs, developing individualized care plans, and implementing interventions to promote health and well-being. You will work closely with patients, families, and healthcare providers to ensure seamless care coordination and effective communication.
Key Responsibilities:- Conduct comprehensive patient assessments and develop care plans that address physical, emotional, and social needs.
- Coordinate services with healthcare providers, community resources, and family members to ensure continuity of care.
- Provide education and support to patients and families to promote self-care and independence.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Collaborate with interdisciplinary team members to share knowledge, expertise, and best practices.
- Current RN licensure in the state of employment.
- Minimum one year of experience as an RN in an acute care setting, with preference for home care experience.
- Valid driver's license and auto insurance.
- Ability to meet the physical demands of the job, including lifting, bending, and standing.
