At Professional Case Management, we are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in providing high-quality care to our clients, who are former nuclear workers with chronic or terminal illnesses.

Key Responsibilities:

  • Conduct in-home nursing visits for clients, assessing their physical and emotional needs.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with a team of RNs, LPNs, and caregivers to ensure seamless care delivery.
  • Provide case management services, including medication management, home safety assessments, and caregiver supervision.
  • Participate in client care conferences and contribute to the development of care plans.
  • Assist with marketing activities, such as visiting with clients and physicians to discuss our programs.

Requirements:

  • Graduate of a state-approved school of professional registered nursing.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two years of nursing experience, including one year in home care or a closely related field.
  • One year of supervisory and/or case management experience preferred.
  • Current CPR certification.
