About the Role
">We are seeking highly skilled and compassionate Support Coordinators or Case Workers to join our team at YSJ Support Services Corp. As a Support Coordinator, you will be responsible for providing exceptional case management services to individuals with disabilities and their families, ensuring their needs are met and exceeded.
">Responsibilities
">- Develop and implement individualized plans for clients, taking into account their unique needs and goals
- Coordinate resources and services to support clients' independence and well-being
- Maintain accurate records and documentation of client interactions and progress
- Collaborate with internal teams and external stakeholders to ensure seamless service delivery
Requirements
">- Bachelor's degree in social work, education/tutoring, or related fields
- 2-3 years of experience in social work, education/tutoring, or related fields
- Proficiency with technology, including Microsoft Office and case management software
What We Offer
">- A competitive salary range of $45,000 - $55,000 per year, based on experience
- A highly flexible schedule allowing for part-time or full-time work based on your preference
- The opportunity to work from home or your preferred location approximately 75% of the time
- The ability to manage your own client caseload size, giving you autonomy and control over your work
