We are seeking a skilled Home Care Clinician to join our HouseCalls team in Oregon.
Job SummaryThis is a rewarding opportunity to provide annual clinical assessments to patients in the comfort of their homes. As a key member of our Home and Community Care team, you will play a vital role in identifying and reducing health risks for patients while coordinating appropriate follow-up care to improve health and well-being.
Key Responsibilities:- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
- Identify diagnoses and develop care management plans to address treatment gaps.
- Communicate findings with primary care physicians to inform patient care.
- Educate patients on disease processes, medication adherence, and self-management techniques.
To be successful in this role, you must have:
- A current NP or PA license, unencumbered and free from any open disciplinary actions.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or equivalent.
- Ability to complete physical demands of the job, including lifting, navigating stairs, and sitting/standing/kneeling.
- Reliable transportation for home visits.
We welcome candidates with experience in gerontology, cardiology, internal medicine, or endocrinology. Familiarity with home healthcare or home visit settings is also desirable.
Benefits and IncentivesWe offer a competitive salary range of $120,000 - $160,000 per year, based on experience and location. Additionally, you may be eligible for student loan repayments of up to $40,000 or a sign-on bonus of up to $30,000, depending on your circumstances.
