About the Role

We are looking for a skilled Nurse Practitioner to provide high-quality patient care in our Coumadin Clinic. The successful candidate will work collaboratively with our multidisciplinary team to assess and manage patients on anticoagulation therapy. This is a part-time position requiring 0.90 FTE commitment.

Key Responsibilities
  • Conduct comprehensive assessments of patients on anticoagulation therapy, including education and treatment for acute and chronic health problems.
  • Develop and implement individualized care plans to ensure optimal patient outcomes.
  • Collaborate with other healthcare professionals to coordinate patient care and share best practices.
Requirements
  • Nurse Practitioner (NP) licensed by the Oregon State Board of Nursing required.
  • Registered Nurse (RN) licensed by Oregon State Board of Nursing required.
  • Experience providing care for clients requiring anticoagulation therapy using Coumadin.
  • Demonstrated ability to analyze problems, recognize causes, devise and implement solutions.
