We are seeking a highly skilled and compassionate Clinical Coordinator I - Social Worker to join our team at the Department of Commerce, Division of Professional Licensing (DOPL). As a Clinical Coordinator I - Social Worker, you will play a critical role in supporting healthcare professionals struggling with substance use disorders.
Key Responsibilities:- Provide case management for assigned participants to facilitate ongoing recovery and successful completion of their monitoring agreement.
- Track participant agreement compliance, including treatment(s), medication(s), and/or toxicology testing.
- Make appropriate and timely chart entries for clinical encounters with participants.
- Complete all forms and documentation in a timely manner, managing the correspondence regarding assigned participants, and ensuring that regular correspondence is sent out to various parties.
- Coordinate toxicology, review check-ins, toxicology testing, and follow-ups with participants; communicate with participants regarding issues with the collection process and/or toxicology results; assist with scheduling make-up toxicology testing as needed.
- Consult with others to discuss treatment plans, new or unusual situations, findings, options, and recommendations on client/patient cases.
- Oversee assigned participant charts and ensure their maintenance, that relevant data is entered in charts in a timely manner and meets UPHP expectations for charting, relevant data is entered, and that policies and procedures are implemented as written.
- Provide general clinical duties involving maintenance of participant communications, both verbal and written, assist in setting up contracts for assigned participants to ensure a smooth transition, and manage participants in crisis, involving UPHP Program Manager and participants care providers, where indicated, and engage in special projects as needed.
- Assist with all aspects of new participant sign-ups, clarifying locations of all state licenses, clinical privileges, and workplace monitor(s), ensure all relevant consents are signed and forms are complete, and complete entry into the electronic records system.
- Understand and follow applicable state and federal laws, as well as UPHP policies, regarding confidentiality and protected health care information and carry out responsibilities in a manner consistent with UPHP policies and procedures.
- Accept referrals to and inquiries about the program, from individuals, places of employment, or other concerned parties; gather information, communicate with referral sources, and complete participant records and the internal processes related to new inquiries.
- Testify in court, or hearings regarding actions, findings, procedures, and protocols, etc.
- Perform outreach in the community to spread awareness about UPHP.
- Demonstrated effective interpersonal skills and the ability to work as part of a clinical team.
- The conviction that substance use disorders are treatable primary diseases, and be amenable to working in an abstinence-based program philosophy.
- Exceptional writing and case management skills.
- Proficiency with Word, Excel, PowerPoint, Google Docs, and Google Sheets.
- An excellent ability to build trust and rapport.
- A self-starter attitude and great work ethic.
- At least one year of clinical experience with individuals with substance use disorders.
- Experience providing case management.
