Job Summary

The Licensed Mental Health Practitioner (LMHP) at Midtown Health Center Inc. provides direct mental and behavioral health services to patients. This role involves assessing patients to determine their needs, referring them to necessary services, and monitoring their progress.

Key Responsibilities
  1. Provide interventions to patients within the scope of your expertise and credentials.
  2. Offer therapeutic interventions, counseling, and education on mental health needs.
  3. Conduct assessments to establish a foundation for treatment plans.
  4. Work with patients on a daily basis, using professional judgment and discretion to ensure they receive necessary services and care.
  5. Assist in developing, implementing, and revising individual or family treatment plans, ensuring services provided align with the plan and monitoring progress toward goals.
  6. Identify patients' insurance coverage and other sources of payment for services.
  7. Address patients' risk factors and obstacles to care.
  8. Connect patients with services and resources to meet their needs and treatment goals.
  9. Communicate patients' care preferences, serving as their advocate, and verifying that interventions meet their needs and treatment goals.
  10. Serve as a liaison between patients and primary care providers to resolve care concerns and collaborate with providers to ensure integrated care.
  11. Consult and cooperate with community systems to facilitate linkage, referral, crisis management, advocacy, and follow-up to attain treatment goals.
  12. Stay up-to-date with new information and materials by reviewing clearinghouse information, newsletters, and periodicals.
  13. Research and design educational materials to assist patients in developing and achieving personal health goals.
  14. Provide supervision to provisionally licensed mental health practitioners as assigned.
  15. Perform other duties as assigned.
Requirements
  • Master's degree in social work, human services, psychology, sociology, or a related field from an accredited college/university.
  • Training in crisis intervention and case management preferred.
  • Experience in community-based behavioral health care and case management preferred.
  • Current, unrestricted Mental Health Provider or Psychologist license in the State of Nebraska.
  • Valid driver's license and ability to use own transportation for work purposes.
