Resumen del Trabajo:

Este puesto se encarga de proporcionar servicios de gestión de casos para los clientes de una organización sin fines de lucro que apoya a veteranos a través de una subvención de Servicios de Apoyo para Familias de Veteranos (SSVF). El candidato ideal debe sentirse cómodo trabajando con veteranos en situación de calle y en desventaja, provenientes de diversos orígenes. Utilizando un conocimiento de los recursos disponibles, esta persona referirá y asistirá al cliente para aumentar sus ingresos y abordar otras barreras hacia una vivienda estable.

Este puesto abarca un área geográfica regional, brindando gestión de casos de bajo contacto a clientes que residen en áreas rurales y urbanas, a través de múltiples condados.

Funciones del Trabajo:

  1. Desarrollar, con la participación y acuerdo del cliente, un Plan de Estabilidad Habitacional (HSP) que aborde la mitigación de las barreras de vivienda. Discutir este plan con cada cliente mensualmente.
  2. Facilitar el logro del bienestar y la autonomía del cliente a través de la defensa, evaluación, planificación, comunicación, educación, gestión de recursos y facilitación de servicios.
  3. Proporcionar referencias adecuadas a otros programas que puedan ayudar a los clientes a alcanzar sus objetivos.
  4. Asistir en la divulgación virtual y en persona a la comunidad para encontrar más hogares de veteranos a los que servir.
  5. Brindar gestión de casos de "bajo contacto", por teléfono o videollamada, mensualmente a los clientes, verificando los problemas de tenencia y el progreso hacia los objetivos. Visitar a los clientes en sus hogares aproximadamente una vez cada seis meses.
  6. Enviar documentación completa y precisa necesaria para que la agencia proporcione asistencia financiera para una parte del alquiler de todos los clientes mensualmente. Monitorear los pagos de alquiler para asegurar el cumplimiento del programa y la adherencia al presupuesto de la subvención.
  7. Mantener archivos completos de los clientes, especialmente notas de caso y documentación de asistencia financiera.
  8. Demostrar las mejores prácticas, incluyendo el enfoque de vivienda primero, la autodeterminación del cliente y la perspectiva de fortalezas.
  9. Mantener un archivo personal de recursos de contactos de agencias de servicios sociales y propietarios en el área para referir a los clientes a los servicios necesarios.
  10. Asistir a reuniones y conferencias, según sea necesario.
  11. Comunicar con los propietarios para identificar y abordar problemas con la tenencia de los clientes. Cultivar relaciones con propietarios que resulten en más oportunidades de vivienda para los clientes de la agencia.

Calificaciones:
  1. Título universitario con dos años de cursos a nivel universitario en un campo relacionado (psicología, sociología) deseable. Se requiere un título de AA/AS en un campo relacionado. Se puede sustituir la experiencia de dos años en el campo de los servicios sociales por educación.
  2. Experiencia trabajando con veteranos y/o personas que experimentan la falta de vivienda preferida. Experiencia trabajando con clientes con adicciones y/o enfermedades mentales preferida. Conocimiento de los servicios sociales locales preferido.
  3. Excelentes habilidades organizativas y de gestión del tiempo.
  4. Demostrada capacidad para trabajar de manera independiente dentro de una organización matricial es altamente deseable.
  5. Dominio de los programas Word, Excel y Outlook, y familiaridad con la entrada de datos en programas de bases de datos es requerida. Se requiere competencia en investigación en internet, plataformas de reuniones virtuales y funciones comerciales en línea y basadas en la nube.
  6. Debe tener un historial de manejo limpio y transporte confiable.
  7. Capacidad para viajar y permanecer durante la noche en partes más distantes del área de servicio, hasta 3 días por mes.
  8. El estatus de veterano es preferido, pero no requerido.
Ofrecimiento:
