Residential Nurse Specialist

We are seeking an experienced Licensed Practical Nurse (LPN) to join our dedicated team in a residential program.
About the Role:
The successful candidate will provide high-quality health services and direct care to individuals with disabilities living in our residential program. This includes advocating for clients' medical needs, monitoring medical records, and administering medications as per protocols. The LPN will also work collaboratively with our healthcare team, residential manager, and other community agencies to ensure seamless care delivery. Additionally, this role involves supporting daily living activities, such as grooming, bathing, feeding, and toileting, while maintaining the dignity and comfort of our residents.
Responsibilities:
- Provide direct nursing care and support to individuals with disabilities, including medication administration, wound care, and vital sign monitoring.
- Document health care services according to state regulations, nursing standards, and Massachusetts LPN license requirements.
- Collaborate with the registered nurse and Director of Health Care to ensure comprehensive care delivery.
- Liaise with community agencies and treatment providers to address regular medical needs and coordinate care.
- Deliver in-service education to staff on health-related topics.
- Maintain knowledge of current policies and procedures related to medication administration.
Requirements:
- Current Massachusetts Nursing license
- Minimum two years of general nursing experience, preferably in a community health setting
- Ability to foster positive relationships with clients, colleagues, and external partners
- Excellent written and oral communication skills
- Basic to intermediate computer-level competency
- Valid driver's license with a good driving record history
