Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in a residential program. As an LPN, you will be responsible for delivering high-quality health services and direct care to individuals with disabilities, ensuring their comfort, safety, and well-being at all times. This role requires strong communication and interpersonal skills, along with the ability to work collaboratively with our healthcare team and external partners. Responsibilities:
- Provide direct nursing care, including medication administration, wound care, and vital sign monitoring.
- Collaborate with the registered nurse and Director of Health Care to develop and implement comprehensive care plans.
- Liaise with community agencies and treatment providers to address regular medical needs and coordinate care.
- Document health care services according to state regulations, nursing standards, and Massachusetts LPN license requirements.
- Deliver in-service education to staff on health-related topics.
Requirements:
- Current Massachusetts Nursing license
- Minimum two years of general nursing experience, preferably in a community health setting
- Ability to foster positive relationships with clients, colleagues, and external partners
- Excellent written and oral communication skills
- Basic to intermediate computer-level competency
- Valid driver's license with a good driving record history
