This position offers a hybrid work model, allowing you to work from anywhere in the continental United States while adhering to California hours from approximately 9:00 AM to 5:00 PM Pacific Time.

Role Responsibilities

The Care Manager (RN) will collaborate with external providers throughout the care continuum to enhance the quality of care for our members. This role involves identifying, developing, coordinating, promoting, supporting, and implementing clinical quality and service improvement initiatives. The Care Manager serves as a resource for integrating evidence-based practices, providing provider training and education, reviewing and developing policies, and interpreting regulatory and accreditation standards. You will manage multiple complex projects that influence the quality and effectiveness of healthcare delivery and services, working alongside external-facing leaders and key stakeholders to support multidisciplinary teams.

Your Focus Areas Will Include:

  • Delivering primarily telephonic post-discharge care coordination and management services, with a focus on Altais' full-risk Medicare population.
  • Conducting clinical assessments and providing education to members, families, and caregivers while developing and implementing personalized care plans.
  • Utilizing advanced clinical judgment and critical thinking skills to execute care management responsibilities.
  • Collaborating with inpatient care managers, Referral Services, Population Health, and other internal teams.
  • Engaging with primary care providers, specialists, and partners from hospitals, skilled nursing facilities, and health plans.
  • Participating in health plan and quality audits as required.

Qualifications and Skills

  • Current RN License (California).
  • Experience in care management coordination and utilization.
  • Familiarity with Epic software is preferred.
  • A minimum of 3 years of experience in a clinical or healthcare setting, demonstrating expertise in quality improvement and clinical program oversight/project management.

Physical Requirements: This role involves working in an office environment, with a combination of in-office and remote work. The position is primarily sedentary, requiring desk work for most of the workday.
