- Document Patient Progress: Accurately document patient progress, including assessments, interventions, and outcomes, to ensure continuity of care.
- Collaborate with Interdisciplinary Team: Work collaboratively with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care planning.
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- License: Current, unrestricted state license as a Registered Nurse in the state of practice.
- Certification: Current CPR certification.
- Experience: One year of experience required working under current nursing license in California.
- Recent Experience: Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home Health Experience: Experience working in home health care settings.
- Communication: Must be able to speak, write, read, and understand English.
- Travel: Must be able to travel to patient homes.
- Lifting: Must be able to lift 50 pounds.
- Physical Demands: Prolonged walking, standing, bending, kneeling, reaching, twisting, sitting, and climbing stairs.
