Job Summary:

We are seeking a skilled Primary Clinician to join our Adult Community Clinical Services (ACCS) program. As a key member of our team, you will work closely with individuals to establish a therapeutic relationship and provide care coordination.

Key Responsibilities:

  • Develop a collaborative relationship with individuals, family, and other providers to ensure comprehensive care.
  • Complete person-centered assessments and service planning to support independent living.
  • Advocate for individuals and coordinate care to ensure seamless transitions.
  • Provide education and support to help individuals connect with community resources.

Requirements:

  • Master's degree in Social Work or Mental Health Counseling required.
  • Licensure required within 2 years of hire (LMHC or LICSW).
  • At least one year of related experience.
  • Excellent written and verbal communication skills, with ability to make effective public presentations.
  • Valid driver's license and reliable transportation.
