Role Overview

The Nurse Care Coordinator plays a crucial role in establishing and nurturing long-term partnerships with healthcare providers, their staff, and Healthmap members engaged in our Kidney Management Program. This position involves coordinating performance enhancement initiatives and care management strategies aimed at elevating health outcomes.

Key Responsibilities
  • Serve as the primary contact between Healthmap, healthcare provider practices, and members to ensure effective engagement and performance within our programs.
  • Forge new partnerships with providers to enhance engagement and manage an assigned caseload effectively.
  • Identify and leverage opportunities to enhance health outcomes for Healthmap members based on specific provider data.
  • Facilitate education and communication regarding best practices, process improvements in provider workflows, and Kidney Health Management interventions.
  • Spot opportunities to educate provider offices on Chronic Kidney Disease, End Stage Renal Disease, and related therapies.
  • Collaborate with physicians and their teams to identify members who could benefit from Care Navigation support, conducting outreach and engaging them in our programs.
  • Provide education to Healthmap members on kidney health and associated co-morbid conditions.
  • Act as an advocate for Healthmap members, utilizing community resources and serving as a liaison among the member, their support network, treating physician, and ancillary providers to help them achieve their individual health goals.
  • Be accountable for personal and departmental performance metrics as established by the organization.
  • Ensure timely and accurate reporting to both internal and external stakeholders.
  • Maintain comprehensive documentation of all interactions with providers and members to ensure consistency and compliance with industry standards.
  • Adhere to Healthmap policies and procedures, ensuring compliance with privacy laws and regulations.
  • Perform additional duties as required.
Qualifications
  • Minimum of an Associate degree in nursing.
  • Active and unrestricted RN license is mandatory.
  • At least 3 years of progressive experience in healthcare services, clinical operations, or care management.
  • Preferred experience includes 3 years in care gap closure or care coordination activities in outpatient or hospital settings.
  • Experience in building and managing relationships with healthcare providers or patients is preferred.
  • A valid and unrestricted driver's license is required, as this role involves regular travel.
  • Candidate must reside in the assigned state.
