Job Title: Nurse Care Coordinator

University of Washington is seeking a skilled Nurse Care Coordinator to join our team. As a key member of our interdisciplinary healthcare team, you will play a vital role in ensuring the smooth transition of patients through our healthcare system.

Key Responsibilities:
  • Identify and monitor high medical needs patients to ensure timely completion of consults, procedures, and tests.
  • Facilitate coordination between providers across the continuum of care.
  • Lead medical team and nursing huddles to remove barriers to discharge due to medical complications and/or system or team obstacles.
  • Assist with ordering complex medical equipment and ensuring safe transitions of care.
  • Develop and monitor outcome measures related to care coordination and staff and patient satisfaction with discharge planning.
Requirements:
  • Master's degree in Nursing or related field.
  • Three years of professional nursing experience.
  • Experience in Case Management.
  • Able to communicate effectively in English, both verbally and in writing.
  • Effective interpersonal skills, leadership skills, and clinical expertise.
  • Ability to access information and document in electronic medical record.
  • Current Washington State RN licensure at time of hire, and current BLS.
