Nurse Care Coordinator

Job Summary:
We are seeking a highly skilled Nurse Care Coordinator to join our team at the University of Washington Medical Center-Montlake. As a Nurse Care Coordinator, you will play a critical role in ensuring the smooth transition of patients through the healthcare system, working closely with the interdisciplinary healthcare team to achieve coordinated patient flow.
Responsibilities:
- Assess and screen patients being admitted to identify high medical needs and develop a plan to ensure timely completion of consults, procedures, and tests.
- Lead medical team and nursing huddles to facilitate coordination between providers across the continuum of care.
- Assist with ordering complex medical equipment and ensuring safe transitions of care.
- Develop and monitor outcome measures related to care coordination and staff and patient satisfaction with discharge planning.
Requirements:
- Master's degree in Nursing or related field.
- Three years of professional nursing experience, with experience in Case Management strongly preferred.
- Able to communicate effectively in English, both verbally and in writing.
- Effective interpersonal skills, leadership skills, and clinical expertise.
- Ability to access information and document in electronic medical record.
- Current Washington State RN licensure at time of hire, and current BLS.
