Job Title: Registered Nurse - Care Management

Harborview Medical Center, a comprehensive healthcare facility owned by King County and operated by UW Medicine, is seeking a skilled Registered Nurse to join our Care Management Team.

Job Summary:

We are looking for a dedicated and compassionate Registered Nurse to coordinate all aspects of in-patient care, leading to a timely transition to the next level of care for patients on acute care units. The ideal candidate will have two years of recent inpatient nursing experience in Trauma/General Surgery, Critical and/or Acute Care, Emergency Care, or equivalent experience.

Responsibilities:
  • Coordinate patient care transitions to ensure seamless and efficient care.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Provide education and support to patients and families to promote self-care and independence.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Current license to practice as a registered nurse in the State of Washington.
  • Two years of recent inpatient nursing experience in Trauma/General Surgery, Critical and/or Acute Care, Emergency Care, or equivalent experience.
  • Case Management or discharge planning experience and understanding of care coordination and transitions of care.
  • BLS certified.
