Job Description
The Clients Support Coordinator will play a critical role in ensuring that clients receive the necessary support and services to achieve their goals. Key responsibilities include developing and updating individualized treatment plans, coordinating between clients and outside resources, and maintaining accurate documentation of client progress and behavior. The ideal candidate will possess strong communication and interpersonal skills, as well as the ability to work independently and collaboratively within a team environment.

Key Responsibilities
- Develop and update individualized treatment plans for each client
- Coordinate with external resources to ensure client needs are met
- Maintain accurate documentation of client progress and behavior
- Interact with various stakeholders, including clinical staff, leadership, and external contacts

Requirements
- 1-3 years of experience in social services, supported housing, or related field
- Ability to work independently and collaboratively within a team environment
- Strong communication and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information

Salary and Benefits
This part-time position offers a competitive salary of $50,000 per year, based on experience, plus additional benefits.
