Registered Nurse Care Coordinator

Experienced Aging Life Care Specialist

Engage directly with clients and their families to deliver support for home health services and community-based care management. Conduct comprehensive evaluations to identify the physical, emotional, social, and spiritual requirements of our clients, and formulate a care strategy that promotes stability, clarity, and peace of mind while overseeing its implementation.

Act as a representative in the community: Participate in local committees, task forces, and boards. Responsibilities may include: educating and presenting to professional groups; raising awareness and assisting professionals in understanding how we can support their clients/patients; organizing meetings with professionals to understand their needs and collaborate on solutions.

This role requires a unique professional who is equally adept at interacting with older clients and their families, as well as with our client's network of support providers, including Trust Officers, Attorneys, Physicians, and other distinguished service providers. We seek an individual who is confident in the significance of Care Management services and embodies the sentiment, 'I’m pleased you reached out, as I am here to assist you.'

The Registered Nurse Care Coordinator will:

  • Oversee all in-home caregivers
  • Conduct routine supervisory visits in accordance with State Regulations
  • Mentor and support caregivers within their designated area
  • Provide education to caregivers, staff, clients, and other stakeholders on clinical matters ranging from disease processes to the execution of hands-on tasks
  • In-Home Nursing
  • The second aspect of this hybrid role necessitates the RN Coordinator to be comfortable and proficient in performing certain in-home nursing tasks.

Responsibilities may include:

  • Wound Management
  • Medication Administration
  • Medication Box Setup and Refill Visits
  • Catheterization Procedures
  • PIV/PICC Dressing Changes
  • Blood Collection

Additional Responsibilities (include but are not limited to)

  • Intake Assessments and Consultations
  • Performance Management
  • Community Outreach and Support
  • Care Management Services

Knowledge and Skill Requirements:

  • Educational Requirement: Registered Nurse
  • A minimum of two (2) years of experience in Nursing
  • Ability to collaborate effectively within a team environment
  • Ability to interact professionally with client committees, including family members, physicians, attorneys, etc.
  • Exceptional written and verbal communication skills are essential.
  • Proficiency in computer skills is required.
  • A valid driver's license and proof of current auto insurance are necessary.
