Physical Therapist Home Health Per Diem

Join our team at CenterWell Home Health, where you'll have the opportunity to make a meaningful impact on the lives of our patients. As a Home Health Physical Therapist, you'll play a vital role in helping patients recover from illness, injury, or surgery, and regain their independence.
Key Responsibilities:- Develop and implement personalized physical therapy treatment plans to help patients achieve their goals.
- Conduct thorough assessments and evaluations to identify patients' strengths and areas for improvement.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and training to patients and their families on proper techniques and strategies for optimal recovery.
- Document patient progress and outcomes to inform treatment decisions and improve care quality.
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
