About the Role

We are seeking a skilled Physical Therapist to join our team at CenterWell Home Health. As a Home Health Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and surgeries, and regain their independence.

Key Responsibilities
  • Develop and implement personalized physical therapy treatment plans for patients in their homes, tailored to their unique needs and goals.
  • Conduct thorough assessments and evaluations to identify patients' strengths and areas for improvement, and create targeted interventions to address these areas.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Provide education and training to patients and their caregivers on proper techniques and strategies for managing chronic conditions and promoting overall health and wellness.
  • Document patient progress and outcomes, and communicate these findings to patients, families, and healthcare teams.
Requirements
  • Master's degree in Physical Therapy from an accredited program.
  • Current and unrestricted Physical Therapy license.
  • Minimum of one year of experience in home health or a related field.
  • Strong organizational and communication skills, with the ability to work effectively in a fast-paced environment.
