About the Role

We are seeking a skilled Occupational Therapist to join our Home Health team on a per diem basis. As a key member of our interdisciplinary team, you will play a vital role in helping patients regain their independence and improve their quality of life.

Responsibilities
  • Assess patients' daily living and work-related skills, and develop personalized therapeutic retraining programs.
  • Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patient function.
  • Collaborate with healthcare team members to develop and revise patient care plans.
  • Provide therapeutic treatment and instruction to patients, as ordered by physicians, to improve strength, coordination, and function.
  • Review and expand retraining programs to meet patients' behavioral goals and rehabilitation program objectives.
  • Consult with vocational team members to develop vocationally oriented plans, as needed.
  • Communicate patient status and progress to physicians, rehabilitation staff, and family members.
  • Participate in care coordination and discharge planning activities, and serve as a resource to other healthcare personnel.
Requirements
  • Master's degree from an accredited Occupational Therapy program.
  • Minimum six months of occupational therapy experience, with home health experience preferred.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
