Key Responsibilities- Develop and implement personalized physical therapy treatment plans for patients in their homes, tailored to their specific needs and goals.
- Conduct thorough assessments and evaluations to identify patients' strengths and weaknesses, and develop targeted interventions to improve their physical function.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Provide education and training to patients and their caregivers on proper techniques and strategies for managing chronic conditions and preventing further injury.
- Document patient progress and outcomes accurately and thoroughly, and communicate effectively with healthcare teams and patients' families.
- Master's degree in Physical Therapy from an accredited program.
- Current and unrestricted Physical Therapy license.
- Minimum of one year of experience in home health or a related setting.
- Strong organizational and communication skills, with ability to work independently and as part of a team.
