Job Overview

We are seeking a skilled Mental Health Recovery Coach to join our Behavioral Health team. Working closely with our interdisciplinary team, you will provide supportive services to members with Serious Mental Illness and Substance Abuse Disorder History, empowering them to achieve their recovery goals.

Key Responsibilities
  • Conduct one-to-one sessions with members, supporting them in identifying and creating personal recovery plans.
  • Collaborate with the primary Behavioral Health Care Manager and community treatment team to determine the steps necessary for members to achieve their goals and self-directed recovery.
  • Complete HRA Housing applications and assist with Supportive Housing placements, escorting members to scheduled housing interviews when necessary.
  • Arrange transportation to and from scheduled PCP Medical Appointments.
  • Facilitate Recovery Dialogues and teach members how to overcome relapse fears.
  • Support members working with assigned Parole Officers, as needed.
  • Participate in BH HARP Team meetings, supervision, discharge planning, and clinical rounds discussions.
  • Accompany HARP members to initial HCBS appointments if the assigned CMA worker is unavailable.
  • Utilize your personal recovery experience to empower individuals served by the program to define and pursue their own strength-based recovery and wellness goals.
Requirements
  • High School graduation or equivalent, along with 5 years of professional Behavioral Health experience, or an equivalent combination of training, education, and experience.
  • Knowledge of the Recovery process and the ability to facilitate recovery using established standardized mental health processes.
  • Computer literacy with Microsoft Office (Word, Outlook, and Teams) and working knowledge of how to document member notes within an electronic medical record system.
Preferred Qualifications
  • Required New York Certification as a Peer Specialist.
  • CASAC - T certification is preferred.

Salary and Benefits

The estimated salary for this position is $51,500.00, with opportunities for professional growth and development within our organization.
