RN, Hospice Case Manager

At Chapters Health System, we are seeking a compassionate and skilled RN, Hospice Case Manager to join our team. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in need of hospice services.
Job Summary:The RN, Hospice Case Manager is responsible for coordinating and managing the nursing care of hospice patients and their families. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Key Responsibilities:- Conduct initial and ongoing nursing assessments to identify patient and family needs
- Develop and implement individualized care plans to meet patient and family goals
- Collaborate with the interdisciplinary team to ensure comprehensive care
- Supervise and educate LPNs, CNAs, and Patient Care Volunteers
- Manage patient data and maintain accurate records
- Current RN licensure in the District of Columbia, State of Maryland, or Commonwealth of Virginia
- At least one year of staff nurse experience in a comparable setting
- Hospice and/or palliative care experience preferred
- BSN degree preferred
- CHPN certification
- Experience working with electronic health records
This role requires the ability to work in a fast-paced environment, utilizing tact, patience, and diplomacy. The RN, Hospice Case Manager will work independently and as part of a team, resolving conflicts and managing multiple tasks and priorities.
