About the Role

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at Capital Hospice dba Capital Caring. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals and their families in our communities.

Key Responsibilities
  • Provide comprehensive nursing assessments, develop and implement individualized care plans, and coordinate care with other healthcare professionals to ensure seamless patient transitions.
  • Supervise and mentor LPNs, CNAs, and Patient Care Volunteers to ensure high-quality patient care and adherence to our organization's standards.
  • Collaborate with physicians, payers, and other stakeholders to ensure timely and effective patient care.
  • Manage and coordinate patient care services, including medication management, wound care, and symptom management.
  • Develop and maintain strong relationships with patients, families, and caregivers to ensure their needs are met and exceeded.
Requirements
  • Current licensure as a Registered Nurse in the District of Columbia, State of Maryland, and/or Commonwealth of Virginia.
  • At least one year of staff nurse experience in a comparable setting/unit, with a strong preference for hospice and/or palliative care experience.
  • Bachelor's degree in Nursing from an accredited nursing program, with a BSN degree preferred.
  • Healthcare Provider BLS (CPR) certification required for practice in the State of Maryland and the District of Columbia.
  • Ability to demonstrate exceptional communication, organizational, and interpersonal skills, with a strong work ethic and attention to detail.
Preferred Qualifications
  • CHPN certification.
  • Experience working in a fast-paced environment with multiple priorities and deadlines.
  • Ability to work independently and as part of a team, with a strong focus on collaboration and communication.
