We are seeking a skilled Registered Nurse to join our Hospice Case Management team at Chapters Health System. As a RN Case Manager, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Key Responsibilities- Conduct thorough nursing assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Supervise and mentor LPNs, CNAs, and Patient Care Volunteers
- Manage patient health information systems and maintain accurate records
- Current licensure as a Registered Nurse in the District of Columbia, State of Maryland, or Commonwealth of Virginia
- At least one year of staff nurse experience in a comparable setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
