Job Summary

We are seeking a highly motivated and experienced Community Outreach Coordinator to join our team at Neighborhood Healthcare. As a Community Outreach Coordinator, you will play a vital role in promoting health education, outreach, and access to resources for patients and non-patients.

Key Responsibilities
  • Develop and implement community outreach strategies to increase healthcare access and promote health education.
  • Collaborate with community partners to provide education, health promotion, and outreach services.
  • Present Neighborhood programs and services to individuals and small groups, including presentations and group sessions.
  • Provide referrals to patients and community members for health and social services, including Cal-Fresh and health insurance referrals.
  • Work with the manager to plan and organize health fairs and other public events for local communities.
  • Travel to community partner and outreach locations to present site enrollment presentations.
  • Track and report on outreach program effectiveness after each outreach activity.
  • Make referrals for patients to health and social service agencies and follow up to ensure services were received.
  • Develop relationships with local healthcare and social service providers to facilitate information sharing and service provisions.
  • Represent Neighborhood in meetings with community partners.
  • Meet weekly goals for outreach/engagement targets and prepare monthly quantitative and qualitative reports on outreach/in-reach efforts.
Requirements
  • Bachelor's degree in a related field or equivalent experience.
  • One year of outreach, education, and/or customer service experience within a healthcare setting or related field.
  • Experience working with diverse populations.
  • Bilingual (English/Spanish) required.
  • Valid CA Driver's License and proof of auto insurance required.

  • Excellent verbal and written communication skills.
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.
  • Ability to assess situations and independently develop a plan of action.
  • Knowledgeable about and experience with local community-based agencies.
  • Ability to successfully manage multiple tasks simultaneously.
  • Excellent planning and organizational ability.
  • Ability to work as part of a team as well as independently.
  • Ability to work flexible shifts.
  • Ability to work with highly confidential information in a professional and ethical manner.
