Aveanna Healthcare LPN/LVN Job Description

Aveanna Healthcare is a leading national provider of home health services to medically fragile children and adults. We are seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our team as a Private Duty Nurse. This is a full-time/part-time position with a focus on providing high-quality patient care in the comfort of our patients' homes.

Responsibilities:
  • Implement physicians' prescribed plans of care for pediatric and adult patients
  • Conduct patient assessments and coordinate care with the healthcare team
  • Administer prescribed medication, treatments, and therapies as ordered by physicians
  • Document all care provided in a timely and accurate manner
  • Provide education and training to patients and their families on disease management, self-care techniques, and prevention strategies
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes
Requirements:
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
