Job Summary:

As a Program Coordinator at Limitless Possibilities, LLC, you will be responsible for the direct supervision of Mental Health Rehabilitation Professionals and adult individuals with disabilities in various settings. Your primary goal will be to ensure the specific needs of each individual are met, providing opportunities for growth, informed choice, and adherence to their service and support plans.

Key Responsibilities:

  • Direct Support: Provide person-centered support services to members, promoting maximum choice and control, and independence.
  • Facilitation of Services: Develop, implement, and review individual service plans, ensuring knowledge and skills are sufficient to fulfill roles.
  • Health and Wellness: Promote the health and wellness of all clients, recognizing the importance of a holistic approach.
  • Organizational Participation: Familiarize yourself with the organizational mission, vision, and values, ensuring alignment with company goals.
  • Documentation: Manage documentation requirements efficiently, maintaining accurate records and reports.
  • Client Empowerment: Enhance the ability of individuals to lead self-determining lives, providing support and information to build self-esteem and assertiveness.
  • Assessment: Utilize formal and informal assessment practices to respond to the needs, desires, and interests of individuals.
  • Advocacy: Demonstrate knowledge of diverse challenges facing individuals, including human rights and client rights.
  • Community and Service Networking: Familiarize yourself with formal and informal supports available in the community, assisting individuals in accessing these resources.
  • Building and Maintaining Friendships and Relationships: Support participants in developing friendships and other relationships, promoting social connections.
  • Communication: Employ effective communication strategies and skills to establish collaborative relationships with individuals.
  • Crisis Intervention: Develop knowledge of crisis prevention, intervention, and resolution techniques, matching approaches to individual circumstances.
  • Professionalism: Pursue knowledge and information necessary to perform job duties, staying up-to-date with industry developments.
  • Vocational, Educational, and Career Support: Provide knowledge and support related to career and education concerns, promoting personal growth.
  • Staff Supervision, Training, and Development: Coordinate and lead competency-based direct support staff training and professional development activities, including coaching and mentoring.
  • On-Call: Be available on-call during the week and on weekends, unless part of an on-call coverage rotation, ensuring seamless support.
