Responsibilities- Perform visits and coordinate client care, serving as the point of contact for all disciplines involved in patient care.
- Oversee the frequency of visits for the episode, consulting with physicians and offices as needed to provide detailed information about patient care.
- Accurately collect OASIS data, ensure medication profiles are up-to-date, and report lab values to physicians in a timely manner.
- Attend weekly case conferences and monthly case manager meetings to stay informed and contribute to patient care discussions.
Registered Nurses (RNs) must meet the following requirements:
- Be currently licensed as a Registered Nurse (RN) in the state of employment.
- A minimum of one year of clinical experience is preferred.
- Demonstrate knowledge and skill in current nursing practice.
- Possess a valid state driver's license and automobile liability insurance.
