About the Role

We are seeking an organized and detail-oriented Patient Care Coordinator to join our team at the Department of Veterans Affairs. As a Patient Care Coordinator, you will be responsible for coordinating patient care, managing scheduling, and maintaining accurate records.

  • Communicate effectively with patients, families, and healthcare teams, providing clear instructions and updates on patient status.
  • Maintain accurate and up-to-date records of patient interactions, progress, and outcomes, adhering to regulatory requirements and departmental policies.
  • Participate in quality improvement initiatives, contributing to the development of policies and procedures that promote best practices in patient care.
  • Engage with interdisciplinary teams to ensure seamless care transitions, addressing patient concerns and preferences.
  • Promote a culture of respect, empathy, and compassion, ensuring a positive and supportive environment for patients, families, and staff.

Requirements:

  • A minimum of 1 year of experience in a healthcare setting, with a strong background in patient care coordination.
  • High school diploma or equivalent required; associate's or bachelor's degree preferred.
  • Strong communication and interpersonal skills, with ability to build rapport with patients, families, and healthcare teams.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Willingness to work flexible hours, including evenings, weekends, and holidays, as needed.
  • Basic Life Support (BLS) certification.
