Patient Care Coordinator

Overview:
The TMC Rehabilitation Services team is seeking a highly organized and detail-oriented individual to join our team as a Rehab Coordinator. This role plays a critical part in ensuring the smooth day-to-day operation of our therapy department, while also supporting the delivery of quality patient care and services.
Responsibilities:
* Perform various administrative tasks to support the efficient operation of the therapy department, including maintaining office supplies, filing systems, and documentation.
* Coordinate transportation and assist with patient care under the direct supervision of licensed therapists and in accordance with all state and federal laws.
* Prepare necessary forms for physician signatures and track documentation to ensure timely receipt and completion.
Requirements:
* High school diploma or equivalent education required.
* One year of experience in a healthcare setting, preferably in rehabilitation, or completion of nursing aid certification program.
* Ability to work effectively in a fast-paced environment and maintain confidentiality.
