Key Responsibilities- Conduct thorough assessments of clients' needs and develop personalized care plans.
- Coordinate and schedule home care services based on clients' requirements.
- Cultivate strong relationships with clients, their families, and healthcare professionals to ensure high-quality care delivery.
- Maintain accurate and up-to-date documentation of all client interactions and caregiver assignments.
- Stay informed about industry trends and best practices in home care services.
Requirements
- Bachelor's degree in a related field or equivalent work experience.
- Minimum of 2 years of experience in home care coordination or a similar role.
- Proven ability to assess clients' needs and develop customized care plans.
- Excellent organizational and time management skills.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Proficient in using computer applications and software for documentation and scheduling.
