Housing Navigator

We are seeking a highly motivated and compassionate Housing Navigator to join our Street Medicine Team. As a Housing Navigator, you will play a critical role in assisting individuals at risk of homelessness, as well as those who are chronically homeless or part of homeless families. Your expertise will empower individuals by identifying and addressing the specific challenges they face on their path towards securing and maintaining stable housing and overall well-being.
Key Responsibilities- Conduct outreach and engage with individuals experiencing homelessness in encampments to build relationships and provide support.
- Establish rapport and trust with clients to understand their needs and goals.
- Perform comprehensive case management assessments and develop plans of intervention to address housing, services, crisis intervention, and continuity of care.
- Provide case management and navigation services to assist clients in obtaining and sustaining stable housing.
- Respond to referrals and client requests for case management assessment and intervention within the required response time.
- Identify permanent housing options for clients in accordance with HUD guidelines.
- Maintain client-related data tracking systems, including case notes and HMIS entries.
- AA or Bachelor's Degree in Social Work, Psychology, Counseling, or related field.
- 1-2 years experience in homeless services or case management.
- Knowledge of Housing Resources, HUD Guidelines, and familiarity with various housing programs.
- Strong communication and relationship-building skills.
- Proficient in utilizing Microsoft Word, Excel, and PowerPoint.
- Understanding of Mental Health and Substance Abuse Services.
- Community outreach and networking skills.
- Crisis intervention and problem-solving abilities.
- Electronic Health Record experience and data entry skills.
