Job Title: RN Clinical Liaison

Constellation Health Services is seeking a skilled RN Clinical Liaison to join our team. As a key member of our hospice care team, you will be responsible for generating referrals and building relationships with healthcare professionals in the Merrimack Valley territory.

Job Summary:

The RN Clinical Liaison will function in a dual capacity, supporting the transition of care and generating referrals for our hospice line of business. This role requires a strong clinical background, excellent communication skills, and the ability to work independently in a fast-paced environment.

Key Responsibilities:
  • Develop and maintain relationships with healthcare professionals, including social workers, case managers, and physicians.
  • Identify and prioritize potential referral partners, including decision-makers within each account.
  • Conduct clinical assessments and evaluations to determine the appropriateness of hospice care for patients.
  • Provide education and support to patients, families, and staff about hospice care and services.
  • Collaborate with the hospice team to ensure seamless care transitions and effective communication.
  • Stay up-to-date on industry developments and changes relevant to hospice care delivery.
Requirements:
  • Current RN licensure and a strong clinical background.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Proficiency in EMR, Word, and Excel programs.
