Care Coordinator

We are seeking an experienced professional to join our clinical team at Riverside Health and Rehabilitation Center. As a Care Coordinator, you will be responsible for assessing resident health status data, creating and updating care plans, and monitoring patient care.
Key Responsibilities:- Audit and analyze resident assessment data to ensure accurate and effective care planning
- Develop and implement personalized care plans that meet individual resident needs and preferences
- Maintain accurate records and reports of resident care to support quality improvement initiatives
- RN License in the state of NJ with current registration
- Proven experience in long-term care MDS coordination
A competitive salary range of $70,000 - $90,000 per year, depending on experience, as well as a $5,000 sign-on bonus paid out over 12 months.
