Job Title: Primary Care Physician

We are seeking a highly skilled and compassionate Primary Care Physician to join our team at Southern Home Care Services, Inc.

About the Role:

This is a full-time clinical physician lead role, under the supervision of the Chief Medical Officer, where you will have several clinical responsibilities, along with some specified administrative duties.

Key Responsibilities:
  • Provide high-quality complex and geriatric care to residents and patients across various settings in partnership with non-physician practitioners and other interprofessional team members.
  • Keep high engagement with facility leadership teams, nurse practitioners/physician assistants, patients, and families.
  • Provide collaborating physician duties for on average of 5 but up to 15 nurse practitioners and physician assistants to support them in their clinical roles.
  • Audit up to 10% of practitioner medical orders and/or documentation for each provider monthly.
  • Assume medical directorships for skilled and/or other senior living facilities, if needed.
  • Help identify new growth opportunities with skilled facilities and/or other senior living facilities in the market/region.
  • Participate in company frontline/backup call processes for timely response to acute changes of condition to prevent avoidable hospital transfers, or to meet other clinical needs.
  • Keep a high focus on quality outcomes, including but not limited to care path compliance and productivity, prevention of avoidable transfers, and decrease in polypharmacy and costs of care.
Requirements:
  • Graduate from an accredited school of medicine and is a Doctor of Medicine.
  • Two or more years of administrative leadership experience in healthcare.
  • Clinical medical experience of seven years or more preferred.
Preferred Qualifications:
  • Board certification in Family Medicine, Internal Medicine, or Geriatrics; qualification in Geriatrics preferred.
  • Practicing physician currently licensed under the provisions of the State Board of Medicine.
  • Active unrestricted licensure to practice in the state of employment.
  • Current Drug Enforcement Administration registration.
