We are seeking a skilled and experienced Registered Nurse Case Manager to join our team at St. David's North Austin Medical Center. As a Registered Nurse Case Manager, you will play a vital role in promoting patient-centered care by coordinating the plan of care for patients, managing length of stay, and ensuring appropriate resource management.
Key Responsibilities:
- Coordinate patient care activities to promote quality outcomes and patient throughput
- Develop and implement safe and effective discharge plans in collaboration with the multidisciplinary team
- Identify patients at risk for adverse outcomes during transition from one level of care/setting to another
- Perform comprehensive assessments of psychosocial, medical, and discharge needs of patients/families
- Collaborate with the multidisciplinary healthcare team to facilitate successful care transitions
Requirements:
- 2+ years' experience in case management or 3+ years' experience in clinical nursing
- Associate Degree in Nursing or Nursing Diploma
- Bachelor's Degree in Nursing – preferred
