Position Overview: Hospice Relationship Manager
MCM & Associates is seeking a motivated and experienced Hospice Relationship Manager to enhance our team. This role is pivotal in fostering and nurturing connections with referral sources across various healthcare settings.
Key Responsibilities:
- Develop and sustain a positive perception of MCM & Associates as a premier provider of hospice services within the community.
- Engage in regular outreach to secure new referrals, aiming to meet established targets with healthcare professionals, including physicians, case managers, and social workers.
- Conduct introductory meetings with patients and their families to provide insights into hospice services and obtain necessary consents for care.
- Facilitate the referral process in collaboration with the admissions team to ensure seamless transitions for patients.
- Establish and maintain strong relationships with both current and prospective referral sources.
- Contribute to strategic planning by identifying opportunities for service enhancements to address unmet community needs.
- Maintain an extensive understanding of community resources and assist referral sources in accessing these resources when hospice services are not applicable.
- Assist with LVN routines and triage visits during daytime hours as needed.
- Perform additional duties as assigned.
Qualifications:
- Bachelor's degree in nursing or a related healthcare field is preferred.
- Current LVN licensure is required.
- A minimum of one year of experience in sales or marketing within the healthcare sector.
- One to two years of experience in hospice or palliative care marketing is preferred.
Job Type: Full-time
Compensation: $40.00 per hour

Experience Requirements:

Work Environment: This position involves travel and engagement with various healthcare facilities.
